HOW TO ENTER: This contest begins at 12:01 A.M. central time (CT) on June 1, 2009, and ends at 11:59 P.M. CT on September 18, 2009. To enter online, send your nomination via the online form at www.fameawards.net or via email to info@fameawards.net. To enter via postal mail, submit your nomination by mailing your entry to FAME Awards, 837 E. Grafton Dr., Nixa, MO 65714. To enter via fax, submit your nomination complete with a cover sheet to 866-878-6651. Entries must be received no later than September 18, 2009. Limit two entries per person or e-mail account. Sponsor is not responsible for lost, late, illegible, or incomplete entries; postage-due mail; or entries not received for any reason. Entries become sole property of Sponsor, and none will be acknowledged or returned. All entries must adhere to the nomination eligibility requirements listed on www.fameawards.net.

PRIZE DESCRIPTION: Seven (7) winners will be randomly drawn from a pool of all valid entries. Drawing will take place on or before October 9, 2009. Visa gift cards in the amount of $100, $50, and $25 will be awarded. The first name drawn will be awarded $100. The second and third names drawn will be awarded $50 each. The fourth, fifth, sixth, and seventh names drawn will be awarded $25 each. Each individual may win only one prize. ALL TAXES ARE THE SOLE RESPONSIBILITY OF THE WINNER. The prize is awarded without warranty, express or implied, of any kind. THE PRIZE IS NOT TRANSFERABLE.

WINNER’S: Winners will be contacted via email on or before October 16, 2009. Prizes will be sent via postal mail no later than October 30, 2009.
